What is causing my Ford Territory to overheat?

My 2004 Ford Territory is overheating, especially when traveling over 60 km/h. What is causing it?

Make sure the coolant is topped up, check for coolant leaks, check the operation of the thermostat, and check the operation of the thermo fan.
